Grace To You Home Health
Building relationships first.
Our Mission
Working Together to Decrease Isolation and Increase Independence.

Grace To You Home Health is an NDIS disability support service provider located in Clyde North Victoria, founded by dedicated nurses who have firsthand seen the need of quality home care in our community.
The support that our team offer in between the nursing procedures, meal preparation and other home tasks can be just as meaningful. There are personal stories, jokes, and plenty of memories exchanged. These shared moments—and the relationships that they help build—are an important part of supporting health and well-being. And they are essential to what we do.

About Rosalie
It’s amazing how childhood experience shape a belief about ourselves, others and the world.
I remember during my childhood years in the Philippines, I used to volunteer at a local hospital and pass out food and toys to young children with disability during school breaks. It was clear to me the patient’s trust put to their nurses and how such care can change a person's life. This life-changing experience set me on the path to becoming a Paediatric Nurse.
Having 15 years of pediatric experience in New Zealand and Australia, I have seen extensive clinical conditions and have dealt with different types of family dynamics. I believe in building meaningful relationships with the young ones receiving medical care and their families suit to their individuality. It is my duty to provide the best tailored care and positively affect the lives of individuals in my community. I believe children are precious in God’s sight hence I’m channeling His grace to you.

About Sharon
I have 10+ years of experience in the aged-care and disability support work. It's always been a passion of mine to add value to people's lives. To see them live it to the fullest and be part of it every single day, brings meaning and satisfaction to my role.
Church, Family and Friends are those people who have great influence over my life. Being a Filipino, who grew up in the Philippines where poverty is common, shaped my worldview that not all things in life are given - they are earned. More importantly, keeps my feet grounded and grateful for the life we are blessed with.
To our clients and partners, I set my heart on to deliver you the kind of support service you deserve - and that is exceptional care!
